      eADMISSIONS - EpiSoft's new secure web solution for hospitals

      We have designed, developed and are supporting the initial implementation and roll-out of a secure website for patients to create and maintain their hospital pre-admission records in collaboration with a leading private hospital. The eADMISSIONS system captures more than 300 administrative and clinical questions from patients in advance of their hospital admission and submits the data electronically to the hospital's Patient Administration System (PAS) and Electronic Health Record (EHR) systems.

      Patients can store their hospital preadmission records online and copy and verify them for subsequent admissions to hospital, saving the many repeat visitors to this large and busy community hospital a significant amount of time filling out paper forms. If patients have a scanner at home or at work, they can also scan and upload other important documents related to their admission such as their consent and their doctor booking letter, resulting in a totally paperless preadmission process. These documents together with data entered by the patients appear on a worklist for clerical staff to process into the PAS after a record matching process. Following import to the PAS, data and documents are immediately available to clinical staff via the hospital's Electronic Health Record in view-friendly print-friendly format.

      The eADMISSIONS system has been extremely well received by patients in the Phase 1 roll-out which has involved promoting the website via a limited number of doctors' rooms. A broad range of patients including maternity, paediatric, same-day and overnight surgical patients have taken to the system enthusiastically. The target take-up rate in the hospital's original Business Case was achieved by the third week of the Phase 1 roll-out. The hospital is now on track to implement the system with all admitting doctors and all interested patients from early 2012. A Phase 2 release of eADMISSIONS allowing data capture of clinical histories for patients admitted via Emergency or direct to ward is now in the pipeline that will integrate seamlessly into the hospital's ePathways / clinical documentation system.

      The patient's clinical history uses the national standard SNOMED-CT for coding current conditions and alleriges. Patients search and select from the MIMS medication list to record their current medications. This feature has resulted in over 90% of medications being submitted as fully coded data. An identical method used to search and select allergies from the national standard SNOMED-CT termset is resulting in significantly less coded clinical terms. The reasons for this will be reviewed and addressed with refinements to the allergy search module as the hospital continues its roll-out.

      As part of the initial PAS and EHR integration testing, an extensive data provenance review was conducted which is ongoing as the hospital integrates an increasing amount of data from the clinical history into its structured Electronic Health Record for review and verification by clinicians.

      A number of other hospitals are progressing towards a Business Cases for adoption of eADMISSIONS.
