EpiSoft Online Help for Client (Patient) Login

      • Overview

        Your health professional may have registered you with EpiSoft to log in and answer an online questionnaire or to provide your consent to a particular research program. If your health professional has registered you with EpiSoft, they will have recorded some personal details about you to enable you to access the system. This will include your name and your contact details including an email address. Your health professional may also register your mobile telephone number for reminders.

        For first time login, use the temporary password sent to you by EpiSoft. If you have not received the temporary password that your health professional sent you via email, please check that your email has not been automatically redirected to a spam or junk email folder. We recommend you set EpiSoft Support as a trusted sender to your email Inbox. We will only ever send requests to enter data that you will be expecting. If you receive an email from EpiSoft Support that you were not expecting, please do not hesitate to contact us immediately.

        The first time you log in you will be asked to change your temporary password to one known only to you. You will also be required to record two challenge questions and answers about yourself. These should be questions and answers known only to you as they will be used to automatically reset your password and send you a new temporary password if you forget the one that you've created. Therefore please choose your challenge questions and answers carefully.

        For second and all subsequent logins, use the password created by you and not the temporary password sent to you via email.

        To log in, go to www.episoft.com.au and select Patient login from the links at the top right of the page.

        Take a few moments to add the EpiSoft Home page or the EpiSoft Patient Login page to your Internet Favourites or Bookmarks so you can navigate back to this page easily. The patient login page is blue and so if you are on a page with a green login box, you are in the wrong place! An interim page will list the clinical communities which EpiSoft support. Please select the one that is applicable to you. Your user name and password will only be valid for a certain clinical community.

        The login page and all subsequent pages of the system are encrypted. The information you enter will be available to your health professional and no other party. Enter your user name and password to access the system. The questionnaires, diaries and consent forms that are available to you are those issued by your health professional and will only be available to you to complete when your health professional has issued you a form to complete. If they have not issued you a form to complete, you may log in at any time and view information you have previously submitted, but you may not have a form to complete.

      • Forgotten password

        If you forget your password, you can retrieve this at any time by selecting the Forgotten Password link from the Login page. This will bring up a dialogue box into which you enter your user name. Your challenge questions will appear and, if you answer these correctly, a temporary password will be emailed to you. Temporary passwords issued via email will always need to be changed the next time you log in. Please make this change as soon as possible.

        If you forget your user name at any time or change your email address, you can contact your health professional and they can advise you of your user name details and also amend your email address.
